After filling in all the necessary form questions, go to the Design section to select a form design.
You can choose between dozens of different templates. And you can always preview how everything looks on the right portion of your screen.

After choosing the suitable language, click the “SAVE” button.

Once you save your custom form, you can now select it on the QR Code Setup page.
